Minors under the age of 18 (students enrolled in school) need a valid work permit if they want to obtain a job. If you do not have a valid work permit, you CANNOT work.
If you would like to request a work permit, the form is attached. You do not need to have the permit filled out until after you've obtained employment. Once you have the work permit request filled out COMPLETELY (by you, a parent, and your employer) you can drop it off with me. I will then email you within 72 hours when it is ready to sign and pick up.

Attendance and grades will be monitored on a regular basis.
If either drop below expectations or is unsatisfactory, students will not qualify, or the work permit can be revoked.
